No sooner had I launched a series of articles on Human Risk, the first of which can be accessed here, than I find this amazing story:

Bond Salesman Who Wasn’t Reveals RBS Human Error

 Was this just a lapse of manegment and HR or are the causes more deep-rooted?
 
In the follow up articles, beginning next week, we will look at how and why such a problem could occur as well as many other aspects of human or people risk.
